About the Role

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at PHSA. As a key member of our healthcare team, you will provide high-quality patient care and work collaboratively with our interdisciplinary team to deliver exceptional healthcare services.

Key Responsibilities
  • Provide direct patient care services, including assessments, diagnoses, and treatment plans, in a trauma-informed and culturally sensitive manner.
  • Collaborate with our healthcare team to develop and implement individualized care plans that address the unique needs of each patient.
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective treatment plans.
  • Work closely with our interdisciplinary team to coordinate patient care and ensure seamless transitions between healthcare services.
  • Provide education and support to patients and their families on health promotion, disease prevention, and management of chronic conditions.
  • Maintain accurate and comprehensive patient records, including documentation of patient interactions, assessments, and treatment plans.
Requirements
  • Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives.
  • Two (2) years' recent experience providing trauma-informed acute or community health nursing care/services to patients with complex physical, psychiatric, and/or substance abuse/misuse disorders.
  • Successful completion of Occupational First Aid II (OFA II) or OFA III techniques.
  • Working knowledge of community health nursing theory, including mental health and addictions, communicable diseases, primary care, and patient-centered care.
  • Knowledge of the BC Mental Health Act and other relevant healthcare policies and procedures.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse patient populations and healthcare teams.
