Position Overview

As a vital member of a collaborative healthcare team, the Clinical Social Services Specialist will deliver essential clinical social work and discharge planning services to patients in the Complex Transitional Care Units. The responsibilities include:

  • Performing thorough social work and psychosocial evaluations on patients, utilizing diverse methodologies in alignment with professional standards and clinical guidelines.
  • Coordinating, developing, documenting, and executing comprehensive care plans for safe hospital discharge and transition back to community or institutional environments.
  • Engaging in interdisciplinary team meetings, educational rounds, and collaborative discussions.
  • Addressing high-risk or vulnerable situations through effective interventions.
  • Offering support to patients and families to facilitate adjustments related to health challenges and resolve intricate interpersonal issues.
  • Educating stakeholders on relevant legislation, including the HCCA and/or Substitute Decisions Act.
  • Delivering targeted therapeutic interventions, supportive counseling, community outreach, and advocacy efforts.
  • Providing guidance on financial matters, legal concerns, workplace injuries, or community resource referrals.
  • Assisting visitors who require hospitalization.
  • Participating in advanced care planning and goals of care discussions through family meetings and facilitated conversations.

Qualifications

Halton Healthcare is committed to recruiting individuals who demonstrate proficiency in their profession and embody our core values of accountability, respect, and compassion.

  • BSW or MSW is required; current registration and membership in good standing with OCSWSSW.
  • Completion of LEAP (Learning Essentials Approaches to Palliative Care) training is preferred or to be completed within one year of hire.
  • Experience in providing social work services within an acute care or post-acute hospital environment.
  • Strong understanding of community resources and proven ability to collaborate effectively with community partners.
  • Familiarity with the Mental Health Act, Consent to Treatment Act, Substitute Decisions Act, and Healthcare Consent Act.
  • Demonstrated ability to maintain an evidence-based practice approach.
  • Previous group experience with this demographic is preferred.
  • Commitment to ongoing professional development.
  • Proficient documentation skills in accordance with college standards.
  • Ability to work independently, demonstrating critical thinking and effective problem-solving skills.
  • Experience in conducting interviews and providing comprehensive psycho-social assessments.
  • Strong knowledge of the Public Guardian and Trustee Office and Consent and Capacity Board.
  • Excellent attendance and employment record.

Hours: Monday to Friday - Day shift and/or evenings (subject to operational requirements)
