We are seeking a skilled Social Worker to join our team at Fellburn Care Centre in Burnaby, BC.
About the Role:The successful candidate will assist residents and their families to adjust to the stress associated with chronic illness/disability, relocation, and institutional life. This will involve completing and updating psycho-social assessments, providing counseling services, and participating in interdisciplinary care conferences.
Responsibilities:- Completes and updates psycho-social assessments of residents and clients
- Provides counseling services to residents/clients, families, staff and groups
- Attends interdisciplinary care conferences and consults with others
- Assists with the development, implementation and review of resident and client care plans
- Determines residents'/clients' eligibility for assistance of a financial nature
- Participates in resident and client discharge planning
- Attends meetings and participates in hospital and community planning, as assigned
The ideal candidate will have a Bachelor's Degree in Social Work from an accredited school of Social Work, two years of recent experience working as a Social Worker in a long term care setting, and current full registration with the BC College of Social Workers.
