We are seeking a skilled Social Worker to join our team at Fraser Health Authority. As a Social Worker, you will play a vital role in providing support and services to residents and their families in our long-term care facilities.
Job SummaryThe successful candidate will be responsible for completing psycho-social assessments, providing counseling services, and participating in interdisciplinary care conferences. You will also assist with the development and implementation of resident care plans, and provide education and support to residents, families, and staff.
Responsibilities- Complete and update psycho-social assessments of residents and clients
- Provide counseling services to residents, families, and staff
- Participate in interdisciplinary care conferences and consult with others
- Assist with the development and implementation of resident care plans
- Provide education and support to residents, families, and staff
- Determine residents' eligibility for financial assistance and make referrals as needed
- Participate in resident discharge planning and attend meetings as assigned
To be successful in this role, you will require a Bachelor's Degree in Social Work from an accredited school of Social Work, and two years of recent experience working as a Social Worker in a long-term care setting. You must also be currently registered with the BC College of Social Workers.
