Role Overview
In the capacity of Regional Primary Care Coordinator, your responsibilities will include supporting the Primary Care Manager and Primary Care Project Manager in the provision of culturally competent, efficient, and high-quality Primary Care Services. The Coordinator will deliver both operational and administrative assistance for the coordination and execution of primary care services and initiatives within Northern First Nations communities, which encompasses:

  • Access to primary health care services in rural, urban, and remote areas
  • Program planning, development, execution, and assessment
  • Initiatives related to the Primary Care Network
  • Collaborative projects with Joint Project Boards (mPCT)
  • Partnership integration efforts with health organizations (Divisions of Family Practice, Northern Health Authority, First Nations Health Service Organizations)
  • Coordination of primary care with FNHA's Mental Wellness and Addiction Teams.

Candidate Profile

  • A Bachelor's degree or a suitable combination of education, training, and experience
  • A minimum of three (3) years of relevant experience, including prior roles in coordination or administration, or an equivalent mix of experience and education.
  • Recent experience in a healthcare setting
  • Familiarity with First Nations communities or organizations, including engagement with political leadership
  • Exceptional interpersonal and communication abilities; capacity to cultivate collaborative partnerships through a team-oriented approach
  • High motivation, a positive outlook, and adaptability in a dynamic environment
  • Strong organizational capabilities and experience in project management; adept at organizing and prioritizing tasks while managing multiple responsibilities
  • Proficient computer skills and comfort with software applications such as spreadsheets, presentations, diagramming, and word processing (e.g., Microsoft Excel, PowerPoint, Visio, and Word)
  • Knowledge of Regional Health Authority and First Nations Primary Care processes, workflows, referral patterns, program delivery models, and electronic medical records.
