Position Overview

The Resource Coordinator for Complex Special Needs plays a pivotal role in ensuring the effective delivery of services to eligible children. Reporting to the Supervisor of Complex Special Needs, this position is dedicated to a person-centered approach, overseeing the creation and execution of evidence-based service plans tailored to individual requirements. The Resource Coordinator is also tasked with the ongoing assessment and evaluation of current services and future possibilities.

Key Responsibilities
  • Engages with individuals and their families to assess current and future service options, linking them to necessary resources while participating in the goal-setting process through the Plan of Care and Individual Support Plan.
  • Develops personalized plans in collaboration with each child, family, and support network to identify specific support needs.
  • Conducts annual reviews of assigned individuals and families, ensuring compliance with required documentation.
  • Completes all mandated updates and reviews by established deadlines.
  • Provides essential resource information to individuals and families.
  • Establishes and nurtures positive working relationships with all stakeholders.
  • Conducts scheduled and unscheduled visits to monitor service provision for assigned individuals.
  • Verifies information and takes appropriate action on any incidents or serious occurrences related to individual care.
  • Participates in clinical consultations, school meetings, and other relevant gatherings concerning the care of assigned individuals.
  • Responsible for financial forecasting and budget management.
  • Reports any areas of risk concerning individuals to the Supervisor.
  • Collaborates with clients and other professionals to resolve issues as needed.
  • Facilitates financial discussions in accordance with departmental policies and procedures.
  • Maintains the accuracy and integrity of client information in compliance with established standards.
  • Attends regular supervision sessions to review case loads and receive guidance.
  • Adheres to all policies and procedures of the organization.
  • Promotes awareness of services by participating in presentations to community groups and other agencies as required.
  • Performs additional duties as assigned.
Qualifications and Skills
  • A minimum of three years of experience in the developmental services sector.
  • A Bachelor's degree in Social Work or a related field is required.
  • Strong interpersonal skills are essential.
  • Demonstrated analytical, problem-solving, critical thinking, and facilitation abilities.
  • Familiarity with services available for children and adults in the developmental sector.
  • Experience in case management is preferred.
  • Knowledge of relevant legislation and regulations is required.
  • Excellent organizational and time management skills to meet deadlines.
  • Ability to thrive in a fast-paced environment while managing multiple priorities.
  • Proficient in Microsoft Office Suite, including Word, Excel, PowerPoint, and Outlook.
  • A valid driver's license and access to a vehicle for business travel are necessary.
Work Environment

A significant portion of the work occurs in various settings, including family homes and requires local travel. Work schedules and priorities are typically established in advance.
