We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team at Horizon Health Network. As a key member of our multidisciplinary team, you will provide holistic care to individuals and groups of patients, working to the full extent of the LPN scope of practice.
Key Responsibilities- Patient Care
- Collaborate with patients and families to develop and implement individualized care plans
- Provide therapeutic relationships and support patients in their recovery planning
- Act as a patient advocate, promoting open communication and professionalism
- Maintain knowledge of treatments, equipment, and supplies pertinent to the service
- Work collaboratively with other team members to ensure quality patient care provision

- Maintain personal certification skills required
- Model professionalism and maintain standards of practice and professional boundaries
- Assist with and support the orientation of new staff
- Assist with educating patient/families/staff/community

- Act as a change agent to enhance quality of care, work environment, and sustainability
- Minimum of 1 year of recent and direct experience in Addiction and Mental Health
- Actively registered with the LPN Association of New Brunswick (ANBLPN)
- Current BLS, Non-Violent Crisis Intervention, ASIST, and Motivational Interviewing certifications
- High level of interpersonal skills and ability to work well within a team and independently
- Demonstrated awareness of assessment tools and treatment modalities
- Demonstrated knowledge of and adherence to relevant legislation and processes
