CarePartners is a leading provider of home health care services, seeking a skilled Visiting Nurse to join our team in the Parry Sound area.
Key Responsibilities- Develop and maintain effective care plans for patients in community health settings
- Provide exceptional nursing services to patients in their homes, including one-on-one care and virtual consultations
- Communicate and collaborate with patients, family members, and interdisciplinary care teams
- Perform other clinical duties as required
- Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
- Current College of Nurses of Ontario (CNO) registration in good standing
- Strong desire and commitment to making a difference in patients' lives
- Exceptional interpersonal skills
- Valid driver's license and vehicle
- Excellent English verbal and written communication skills
- CPR Certification
- Current immunization record, including 2-step TB test
- Vulnerable sector check and clear background check
