CarePartners is a leading provider of home health care services in Ontario, seeking a skilled Visiting Nurse to join our team. As a Visiting Nurse, you will play a vital role in delivering high-quality patient care in the comfort of their own homes.
Key Responsibilities- Develop and maintain effective care plans for patients receiving community health services
- Provide exceptional nursing services to patients in their homes, including one-on-one care and virtual consultations
- Communicate and collaborate with patients, family members, and interdisciplinary care teams to ensure seamless care delivery
- Perform other clinical duties as required, including administrative tasks and documentation
- Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
- Current College of Nurses of Ontario (CNO) registration in good standing
- Strong desire and commitment to making a difference in the lives of patients
- Exceptional interpersonal skills and ability to work independently
- Valid driver's license and vehicle for regular travel
- Excellent English verbal and written communication skills
- CPR Certification and current immunization record, including 2-step TB test
- Vulnerable sector check and clear background check
