Primary Care Nurse

We are seeking a highly skilled and dedicated Registered Nurse to join our team as a Primary Care Nurse. As a Primary Care Nurse, you will play a vital role in providing high-quality patient care and contributing to the development of our healthcare services.
Key Responsibilities- Provide comprehensive nursing care to patients in primary care settings, including physician practices, homes, schools, and communities.
- Develop and implement individualized care plans in collaboration with the interprofessional team.
- Conduct initial and ongoing client assessments, and provide nursing care through appropriate interventions.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Participate in quality improvement initiatives and contribute to the development of healthcare policies and procedures.
- Registration with the BC College of Nurses and Midwives as a practicing RN or RPN registrant.
- Three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings.
- Current valid driver's license.
