Primary Care Nurse

We are seeking a highly skilled and dedicated Registered Nurse to join our team as a Primary Care Nurse. As a Primary Care Nurse, you will be responsible for providing high-quality patient care in a variety of settings, including physician practices, homes, schools, and communities.
Key Responsibilities- Provide clinical follow-up care to patients in accordance with their primary care plan and personal goals of care.
- Develop and implement individualized care plans in collaboration with the interprofessional team.
- Provide direct clinical care and services to individuals, groups, and communities.
- Collaborate with other healthcare professionals to ensure seamless care transitions and continuity of care.
- Participate in quality improvement initiatives to enhance patient outcomes and experiences.
- Registration with the BC College of Nurses and Midwives as a practicing RN or RPN registrant.
- Three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings.
- Current valid driver's license.
- Competitive hourly wage range: $41.42 - $59.52.
