Key Responsibilities

  • Conduct visits to family residences and community settings within the designated service areas for routine and surprise monitoring assessments. Travel expenses may be reimbursed according to company policy.
  • Collaborate with case management teams to create and execute detailed transition strategies that facilitate successful moves for individuals, whether transitioning from child to adult services or exiting foster care.
  • Design and implement training programs based on competencies and coordinate professional support services.
  • Engage with youth and families to formulate goal-oriented service plans while ensuring meticulous management of client records, including prompt documentation of client interactions.
  • Evening and occasional weekend availability is necessary, as these times are often when families are together and may experience heightened stress.
  • Adhere to CARF/COA accreditation standards and legal requirements.
  • Link children and families with support systems, external agencies, and community resources while organizing continuous service provision.
  • Assist caregivers in developing emergency response strategies, safety protocols, and contingency plans, along with providing crisis intervention services as required.
  • Employ standardized assessment tools and monitoring techniques to maintain procedural consistency and follow-up integrity.
  • Review and discuss recommendations and action plans with caregivers.
  • Prepare incident reports as necessary.
  • Keep current contact notes and program documentation.
  • Participate in multidisciplinary case management discussions, as well as quality enhancement and risk management committees.
  • Perform office-related tasks such as data entry and filing.
  • Undertake miscellaneous job-related responsibilities as needed.

Ideal Candidate Qualifications

  • A minimum of 2 years of relevant experience in case management, particularly with foster families.
  • A College Diploma in Social Work or a related Human Services field.
  • Knowledge and experience with accreditation standards for monitoring foster homes and Child in Care programs.
  • Preference will be given to candidates who have completed the SAFE (Structured Analysis Family Evaluation) home study course and possess relevant experience; however, candidates without this certification must complete the 2-day training during their probationary period.

Additional Qualifications:

  • A valid CPR / First Aid certification (or willingness to obtain during the probationary period).
  • A valid Non-Violent Physical Crisis Intervention Certificate (MANDT) and Conflict Resolution Certificate.
