Position Overview

The Clinical Pharmacist is accountable for delivering comprehensive Clinical Pharmacy services aimed at identifying and mitigating drug-related issues through meticulous medication profile assessments, patient evaluations, and the formulation and execution of Pharmaceutical Care Plans. This role involves providing critical drug information to a variety of healthcare professionals, including physicians and nursing staff, while actively participating in both internal and external committees, conducting Drug Utilization Evaluations, and facilitating educational sessions for professional staff.

Qualifications and Experience
  • Preferred Doctorate of Pharmacy degree (Pharm D).
  • Certification of competency from the Pharmacy Examination Board of Canada.
  • Current licensure as a Pharmacist with the Ontario College of Pharmacists.
  • Completion of an accredited Hospital Pharmacy Residency Program or a comparable combination of education, training, and experience is preferred.
  • Prior experience in a hospital environment with direct involvement in Pharmaceutical patient care is advantageous.
  • Extensive knowledge in therapeutic drug monitoring.
  • Strong understanding of the clinical pharmacist's role within a multidisciplinary healthcare team.
  • Thorough knowledge of drug composition, usage, and clinical effects.
Key Responsibilities
  • As a Clinical Pharmacist, your responsibilities will include:
  • Delivering pharmaceutical care to enhance drug utilization, identifying potential and actual drug-related issues, making informed recommendations, and monitoring drug efficacy and toxicity to achieve optimal patient outcomes.
  • Providing drug information and educational resources to patients and healthcare professionals.
  • Collaborating with physicians and other healthcare providers to develop, implement, and monitor therapeutic strategies.
  • Assisting in the creation of drug therapy guidelines tailored to specific disease conditions.
  • Engaging in medication use evaluations, quality improvement projects, and committee participation.
  • Offering mentorship and guidance to pharmacy students and new staff members.
  • Ensuring accurate validation and dispensing of prescriptions; addressing medication safety concerns by collaborating with the healthcare team to resolve discrepancies promptly; documenting professional practice activities proficiently.
