Position Overview

The Clinical Pharmacist is accountable for delivering comprehensive Clinical Pharmacy services aimed at mitigating and preventing medication-related issues. This role involves thorough medication profile assessments, patient evaluations, and the development and execution of Pharmaceutical Care Plans. The Clinical Pharmacist will also provide essential drug information to healthcare professionals, including physicians and nursing staff, while actively participating in various committees and conducting Drug Utilization Evaluations.

Qualifications and Experience
  • Preferred: Doctorate of Pharmacy degree (Pharm D).
  • Certification of competency from the Pharmacy Examination Board of Canada.
  • Must be licensed as a Pharmacist with the Ontario College of Pharmacists.
  • Completion of an accredited Hospital Pharmacy Residency Program or a similar combination of education and experience is preferred.
  • Experience in a hospital environment with direct involvement in Pharmaceutical patient care is preferred.
  • Strong expertise in therapeutic drug monitoring.
  • Comprehensive understanding of the clinical pharmacist's role within a multidisciplinary team.
  • In-depth knowledge of drug composition, usage, and clinical effects.
Key Responsibilities
  • Deliver pharmaceutical care to enhance drug utilization, identify potential and actual drug-related issues, and monitor drug efficacy and toxicity to achieve optimal patient outcomes.
  • Provide drug information and education to patients and healthcare professionals.
  • Collaborate with physicians and healthcare providers to formulate, implement, and monitor therapeutic strategies.
  • Assist in creating drug therapy guidelines tailored to specific disease conditions.
  • Engage in medication use evaluations, quality improvement projects, and committee participation.
  • Mentor and supervise pharmacy students and new staff members.
  • Ensure accurate validation and dispensing of prescriptions; address medication safety concerns by collaborating with the healthcare team to resolve discrepancies promptly; document professional practice activities effectively.
