Job Title: Partnership & Outreach Coordinator

WoodGreen is seeking a highly motivated and experienced Partnership & Outreach Coordinator to join our team. As a key member of our organization, you will be responsible for developing and implementing an outreach strategy to promote our programs to key stakeholders, including shelters, schools, educational institutions, social service providers, and community organizations.

Key Responsibilities:
  • Develop and implement an outreach strategy to promote our programs to key stakeholders.
  • Educate and inform referring agencies and potential participants on program eligibility criteria, program dimensions, determinants for success, and application process.
  • Utilize knowledge of shelters programs and their protocols to promote our programs and provide strategies to assist shelter staff to assess suitability of women for our program.
  • Coordinate outreach, applications, and intake process to ensure that housing vacancies are filled in a timely fashion, subject to restrictions imposed by unit size and occupancy parameters.
  • Develop and maintain an updated inventory of contacts for key stakeholders on the project, including key personnel at shelters, schools, education institutions, social service providers, community organizations, and previous graduates of the program.
  • Oversee all Homeward Bound Applications and ensure that the program's database and tracking sheets are up-to-date and all pending participants' status is current.
  • Plan, organize, and coordinate information sessions on a monthly or as-needed basis.
  • Coordinate an academic assessment for all applicants and track the status of intake interviews to ensure efficient and timely intake process.
  • Provide referrals and advocacy for applicants by writing support letters and participating in case conferences with other institutions such as CAS, OW, shelters, etc.
  • Assist with coordination of supports and services needed by newly accepted participants.
  • Actively participate in the case management team bi-monthly meetings.
  • Prepare and submit monthly program reports and statistics, which outline activities and accomplishments of the position and program.
  • Build partnerships and maintain relationships with different stakeholders, such as referring agencies and participants, to ensure that best practices are followed.
  • Develop and implement the program's newsletter.
  • Assist with the development and update of an operational manual, which includes program protocols, policies, and procedures for all elements of the program model that would allow for easy replication of the program delivery by third parties.
  • Responsible for following up with past participants and Alumni Council to ensure best practices for outreach and intake process are in place.
  • Recruit alumni for outreach and intake activities, as well as engage them as mentors to current program participants, ambassadors with potential funders and donors, and advisors to enhance current program delivery.
  • Works with other WoodGreen units and staff to ensure ongoing coordination within the Homeward Bound program.
  • Represents Homeward Bound Toronto in developing and implementing Homeward Bound Community of Practice goals and objectives.
  • Participates in regular supervisory sessions.
  • Performs other related duties as required from time to time.
Requirements:
  • Bachelor's degree in Social Work or in a related field.
  • Minimum two (2) years of active outreach work experience, partnership development, and experience related to service coordination with shelters, social service agencies, schools, recreational centers, libraries, and community organizations.
