Job Summary

Vancouver Coastal Health is seeking a skilled Community Liaison Worker to join their Integrated Care Team. As a Community Liaison Worker, you will provide support services to clients with mental health and substance use issues, working across multiple sites and clinics within an integrated health service delivery model.

Key Responsibilities

  • Provide support services to clients with mental health and substance use issues
  • Work across multiple sites and clinics within an integrated health service delivery model
  • Monitor clients, document, and provide verbal reports on observations
  • Facilitate the use of community services and resources
  • Liaise and coordinate services with mental health and substance use professionals
  • Plan, organize, and facilitate groups as required
  • Act as an advocate for clients and provide information and support related to complex healthcare needs

Qualifications

Education and Experience

  • Certificate from a recognized Health Care Worker program, plus two years' recent related experience supporting clients through a holistic mental health, substance use, and primary care lens
  • Valid driver's license and access to a personal vehicle for business-related purposes

Knowledge, Skills, and Abilities

  • Knowledge of community, social, and health resources
  • Ability to keyboard 5000 keystrokes per hour
  • Knowledge of substance misuse and behavior management
  • Knowledge and experience in providing culturally safe and competent care, specifically pertaining to Aboriginal people
  • Knowledge and experience in Trauma-Informed Practice, Harm Reduction, and Recovery-Oriented Care
  • Knowledge of mental health issues and treatment
  • Knowledge of community, social, and mental health resources in the DTES
