Job Summary

Vancouver Coastal Health is seeking a Community Liaison Worker to join our Substance Use Intensive Case Management Team in Vancouver, BC. As a Community Liaison Worker, you will play a vital role in supporting clients with mental health and substance use issues, working collaboratively with an integrated health service team and delivery model.

Key Responsibilities
  • Support clients with mental health and substance use issues, providing guidance and resources to help them achieve their goals.
  • Work collaboratively with an integrated health service team and delivery model, providing support services to clients and facilitating access to community resources.
  • Monitor client progress, document observations, and provide verbal reports as required.
  • Facilitate the use of community services and resources, providing information and support to clients.
  • Liaise and coordinate services with mental health and substance use professionals, ensuring seamless care for clients.
  • Plan, organize, and facilitate groups as required, participating in case planning and assisting clients with daily living activities.
  • Act as an advocate for clients, providing information and support related to complex healthcare needs.
  • Establish and maintain therapeutic relationships based on respect and trust.
Qualifications

To be successful in this role, you will require:

  • A certificate from a recognized Health Care Worker program, such as a Community Health Worker Program, plus two years' recent related experience supporting clients through a holistic mental health, substance use, and primary care lens.
  • A valid driver's license and access to a personal vehicle for business-related purposes, as required.
