Position Overview:
LOFT Community Services is a prominent charitable organization dedicated to assisting individuals facing complex challenges, including mental health issues, substance use, poverty, and homelessness. As one of Ontario's largest providers of mental health services, we are seeking a Supportive Housing Case Coordinator to join our team on a full-time permanent basis.
Role Summary:In this role, you will either work independently or collaborate with a team to create a comprehensive support network for clients living with mental health conditions, addictions, and physical health challenges. Your primary goal will be to empower clients, enhancing their independence and dignity within a secure environment. You will provide direct assistance in various areas, including daily living skills, crisis intervention, and connecting clients to essential community services.
Key Responsibilities:- Develop individualized support and recovery plans in collaboration with clients and other professionals, focusing on their unique priorities and goals.
- Encourage personal empowerment through coaching and mentoring, assisting clients in daily living activities and life skills such as hygiene, medication management, and budgeting.
- Establish and maintain a robust support network, coordinating care plans that involve social, medical, and community resources.
- Provide crisis intervention and develop crisis management plans tailored to individual client needs.
- Represent the agency professionally while promoting program services to clients, families, and external partners.
- Facilitate conflict resolution among clients and between clients and their neighbors, ensuring a harmonious living environment.
- Organize and participate in social events aimed at reducing client isolation and fostering community engagement.
- Maintain accurate records and documentation as required by program protocols.
- Contribute to team efforts by offering support and insights during meetings and collaborative initiatives.
Essential:
- A degree in human services or a related field, with a minimum of three years of experience working with similar client populations.
- Understanding of concurrent disorders and harm reduction strategies.
- Familiarity with the challenges faced by individuals with mental health issues, addictions, and homelessness.
- Ability to build therapeutic relationships and negotiate client goals effectively.
- Knowledge of local community resources and services.
- Strong interpersonal, communication, and counseling skills.
- Proficient in crisis intervention and problem-solving.
- Computer literacy for maintaining client records and statistical data.
- Excellent organizational and time management skills.

- Physical capability to perform tasks requiring manual handling.
