Position Overview

As a vital component of the Critical Care Interprofessional team, the Clinical Social Services Specialist will deliver essential psychosocial assistance to patients and their families.

Key Responsibilities
  • Perform thorough social work and psychosocial evaluations on inpatients, utilizing diverse methodologies in line with professional standards and clinical protocols.
  • Coordinate, design, document, and execute the care strategy for hospital discharge and ensure a secure transition back to community or institutional environments.
  • Engage in team meetings, educational interdisciplinary rounds, and training sessions.
  • Act to mitigate high-risk or vulnerable circumstances.
  • Offer support to patients and families to facilitate adjustment to health-related challenges and resolve intricate interpersonal issues.
  • Educate on the Health Care Consent Act and/or Substitute Decisions Act.
  • Deliver selected therapeutic interventions, supportive counseling, community outreach, and advocacy.
  • Provide guidance on financial matters, legal concerns, workplace injuries, or community referrals.
  • Assist visitors requiring hospitalization.
  • Participate in advanced care planning and discussions regarding goals of care through family meetings and mediated dialogues.
Qualifications

Halton Healthcare seeks to recruit individuals who are proficient in their profession and embody our mission, vision, and values of accountability, respect, and compassion that guide our behaviors and attitudes.

  • Bachelor's or Master's degree in Social Work required.
  • Current registration and membership in good standing with the appropriate regulatory body.
  • Experience delivering social work services within an acute care hospital environment.
  • Strong clinical competencies in assessment, counseling, and discharge planning/community integration.
  • Comprehensive knowledge of community resources and proven ability to collaborate effectively with community partners.
  • Familiarity with relevant legislation such as the Mental Health Act, Consent to Treatment Act, Substitute Decisions Act, and Health Care Consent Act.
  • Demonstrated commitment to maintaining an evidence-based practice approach.
  • Previous group experience with this demographic is advantageous.
  • Commitment to ongoing professional development.
  • Ability to document in accordance with regulatory standards.
  • Capacity to work independently and demonstrate critical thinking and effective problem-solving skills.
  • Proficient interviewing skills and ability to conduct comprehensive and timely psychosocial assessments.
  • Strong understanding of the Public Guardian and Trustee Office and Consent and Capacity Board.
  • Excellent attendance and employment record.
Additional Information

Hours: Monday to Friday - Day shift and/or evenings (subject to operational requirements).

Site: Tri-site.

Salary: Competitive hourly rate.
