Job SummaryWe are seeking a skilled Rehabilitation Case Manager to join our team. As a Rehabilitation Case Manager, you will be responsible for coordinating rehabilitation services for clients and acting as a liaison for the rehabilitation team. You will attend home visits, family meetings, and medical appointments to ensure consistency of information sharing and follow-through with medical recommendations.
Key Responsibilities- Coordinate rehabilitation services for clients, including referrals to other healthcare professionals and community resources.
- Attend home visits, family meetings, and medical appointments to ensure consistency of information sharing and follow-through with medical recommendations.
- Complete paperwork on behalf of clients to seek reimbursement for medical and rehabilitation expenses.
- Provide detailed, comprehensive reports as part of the medical-legal system.
- Masters Degree in Social Work, Occupational Therapy, or Nursing.
- Current registration and membership in good standing with a regulated college.
- Minimum 2 years of experience in case management and/or family counseling.
- Valid driver's license and access to a vehicle.
