Job Title: Rehabilitation Case Manager

We are seeking a highly skilled and compassionate Rehabilitation Case Manager to join our team. As a key member of our multidisciplinary rehabilitation team, you will play a vital role in coordinating rehabilitation services for our clients and ensuring their needs are met.

Key Responsibilities:
  • Coordinate rehabilitation services for clients, including attending home visits, family meetings, and medical appointments
  • Act as a liaison for the rehabilitation team, ensuring effective communication and collaboration
  • Coordinate service referrals and community resources as required, including referrals to other healthcare professionals and setting up transportation services
  • Complete paperwork on behalf of clients to seek reimbursement for medical and rehabilitation expenses
  • Provide detailed, comprehensive reports as part of the medical-legal system
Requirements:
  • Masters degree in Social Work, Occupational Therapy, or Nursing
  • Current registration and membership in good standing with a regulated college
  • Minimum 2 years of experience in case management and/or family counseling
  • Valid driver's license and access to a vehicle
