About the Role

We are seeking a highly skilled and compassionate Rehabilitation Specialist to join our team at FunctionAbility Rehabilitation Services. As a key member of our multidisciplinary team, you will play a vital role in providing expert assessment and treatment services to individuals who have suffered traumatic and acquired neurological injuries and severe orthopedic injuries.

Key Responsibilities
  • Coordinate rehabilitation for clients and act as a liaison for the rehabilitation team
  • Attend home visits, family meetings, and medical appointments to ensure consistency of information sharing and follow through with medical recommendations
  • Coordinate service referrals and community resources as required, including referrals to other healthcare professionals, setting up transportation services, and pharmacy accounts
  • Complete paperwork on behalf of clients to seek reimbursement for medical and rehabilitation expenses
  • Provide detailed, comprehensive reports as part of the medical-legal system
Requirements
  • Masters Degree in Social Work, Occupational Therapy, or Nursing
  • Current registration and membership in good standing with a regulated college
  • MVA experience
  • Client-centered approach and ability to collaborate with a multidisciplinary team
  • Strong verbal and written communication skills and ability to work with different personalities
  • Valid driver's license and access to a vehicle
  • Case management and/or family counseling experience is an asset
