Job Overview

PROVIDING EXCEPTIONAL CARE IN A SUPPORTIVE ENVIRONMENT
The Nurse Practitioner for NPSTAT is tasked with enhancing care delivery to minimize hospital transfers from long-term care facilities by offering direct medical services to residents and collaborating with long-term care homes in a specified region.

This role involves attending to patients at a designated long-term care facility.

Key Responsibilities:
1. Oversee the implementation of high-quality care programs in assigned long-term care establishments.
2. Operate within the full scope of practice, autonomously diagnosing, ordering, and interpreting diagnostic tests, as well as prescribing medications in accordance with NP regulations to prevent unnecessary emergency visits and hospitalizations.
3. Conduct thorough patient evaluations, diagnoses, treatments, and manage pain and symptoms effectively.
4. Foster collaborative relationships with long-term care staff, Ontario Health atHome Care Coordinators, physicians, hospital personnel, and other stakeholders.
5. Address complex patient issues that require advanced clinical judgment.
6. Collaborate with physicians to ensure patient-centered care is maintained.
7. Provide valuable insights and guidance to Ontario Health atHome staff in managing challenging and atypical cases.
8. Support the initiatives of Ontario Health atHome and facilitate their practical application, including Behavioral Supports Ontario.
9. Assist in data collection and program evaluation aligned with NPSTAT goals.
10. Develop and implement training materials and strategies to enhance the clinical skills and capabilities of long-term care staff in managing clinical complexities.
11. Mentor and orient new Nurse Practitioners and support other clinical initiatives as directed by NP leadership.

Required Qualifications:

• Master’s degree in a relevant field.

• Current RN(EC) registration with the College of Nurses of Ontario, including an NP certificate in Primary Health Care or Adult Care.

• Familiarity with the role of Ontario Health atHome in the long-term care sector.

• Minimum of 3 years of nursing experience as an RN.

• A genuine commitment to improving the lives of long-term care residents.

• Strong critical thinking abilities.

• Proven leadership skills with the capacity to work independently as well as collaboratively.

• Collaborative and empathetic approach to patient care.

Preferred Qualifications:

• Gerontology Nurse Certification (GNC) is preferred.

• Experience in emergency departments, mental health, dementia-related behaviors, or geriatrics is advantageous.

• Strong clinical background is preferred.

Work Schedule:
Monday to Friday, 10am-6pm, subject to changes as per the collective agreement (35 hours per week; 70 hours per bi-weekly pay period).

Location and Travel:
Regular travel within the designated region may be necessary. A valid Ontario driver’s license and access to a vehicle are required.
