ParaMed is seeking a skilled Registered Practical Nurse (RPN) to join our team as a Community Care Specialist. As an RPN, you will play a vital role in providing high-quality patient care in various community settings. With the support of RNs for more complex patients, you will work with patients of all ages, delivering care that is tailored to their unique needs.

What You'll Do:
- Participate in scheduled visits in the community to assess patient needs and deliver care within your scope of practice.
- Provide safe and ethical nursing care, using appropriate assessment tools and techniques to meet patient needs.
- Execute and continuously monitor care plans, adjusting as required to ensure optimal patient outcomes.
- Engage patients using a collaborative approach, building rapport and trust to promote positive health outcomes.
- Maintain and update professional patient records related to patient encounters and plans.
What You Bring:
- Registration with the CNO as a Registered Practical Nurse in good standing.
- A valid driver's license and access to a vehicle, with experience in delivering community care an asset.
- Reliability, flexibility, and a willingness to travel to and from patient residences within a designated area.
- Experience working with vulnerable populations, with a commitment to providing compassionate care.
