ParaMed is seeking a skilled Registered Practical Nurse (RPN) to join our team as a Community Care Specialist. As an RPN, you will play a vital role in providing high-quality patient care in various community settings. What You'll Do:

  • Participate in scheduled visits in the community to assess patient needs and deliver care within your scope of practice.
  • Provide safe and ethical nursing care, using appropriate assessment tools and techniques to meet patient needs.
  • Execute and continuously monitor care plans, adjusting as required to ensure optimal patient outcomes.
  • Engage patients using a collaborative approach, building rapport and trust to promote positive health outcomes.
  • Maintain and update professional patient records related to patient encounters and plans.

What You Bring:

  • Registration with the CNO as a Registered Practical Nurse in good standing.
  • A valid driver's license and access to a vehicle, with experience in delivering community care an asset.
  • Reliability, flexibility, and a willingness to travel to and from patient residences within a designated area.
  • Experience working with vulnerable populations, with a commitment to providing compassionate care.
