Key Responsibilities:

  • Participate in scheduled visits in the community to assess patient needs and provide care within your scope of practice.
  • Deliver safe and ethical nursing care, using appropriate assessment tools and techniques to meet patient needs.
  • Execute and continuously monitor care plans, adjusting as required to ensure optimal patient outcomes.
  • Engage patients using a collaborative approach, building rapport and trust to promote positive health outcomes.
  • Maintain and update professional patient records related to patient encounters and plans.

Requirements:

  • Registration with the CNO as a Registered Practical Nurse in good standing.
  • A valid driver's license and access to a vehicle are required.
  • Experience in delivering community care is an asset.
  • Reliability and flexibility are essential for this role.
  • Willingness to travel to and from patient residences within a designated area.
  • Experience working with vulnerable populations is an asset.
