Job Summary

We are seeking a highly skilled Social Worker II to join our Primary Care team at Northern Health Authority. As a key member of our interprofessional team, you will provide comprehensive assessment, care planning, and intervention services to individuals and families in our community.

Key Responsibilities
  • Conduct initial and ongoing assessments to identify client needs and develop effective care plans
  • Provide direct psychosocial care, counseling, and group work to promote client well-being
  • Collaborate with healthcare providers and community resources to ensure seamless care coordination
  • Develop and maintain relationships with clients, families, and community partners to promote person-centered care
Requirements
  • Bachelor's Degree in Social Work from an accredited university
  • Two years recent related experience in healthcare or a combination of education, training, and experience
  • Current registration with the British Columbia College of Social Workers
  • Valid BC driver's license
