Therapist - Patient Care Coordinator

Lifemark Health Group is seeking a highly skilled Occupational Therapist to join our team in Vancouver, BC. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality patient care and contributing to the success of our organization.
Key Responsibilities- Conduct comprehensive assessments of patients' physical and cognitive functioning to identify areas of need and develop personalized treatment plans.
- Collaborate with patients, families, and healthcare professionals to educate and empower individuals to achieve optimal health and well-being.
- Develop and implement individualized treatment plans that address patients' physical, emotional, and cognitive needs.
- Provide expert guidance and support to patients and families to promote independence and self-management.
- Participate in case conferences and case management to ensure seamless care coordination and communication.
- Supervise and mentor Occupational Therapy students to foster professional growth and development.
- Contribute to the development and implementation of clinical programs and services that align with the organization's strategic objectives.
- Registration with the provincial College of Occupational Therapists.
- Minimum 3-5 years of experience in treating patients with physical, neurological, and/or mental health conditions.
- Demonstrated commitment to ongoing professional development and staying current with best practices in occupational therapy.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Strong analytical and problem-solving skills, with the ability to think critically and make sound judgments.
