Job Summary

This is an exciting opportunity for a Registered Nurse to join our team at Ridge Meadows Hospital in Maple Ridge, BC. As a Perinatal Transition Line Registered Nurse, you will have the chance to consolidate your skills in a specialty nursing unit and participate in post-graduate course work applicable to the relevant specialty nursing unit.

Responsibilities
  1. Assess patients and collect information from various sources using observation, communication, data review, and physical assessment skills.
  2. Collaborate with patients and their families/caregivers to develop care plans and encourage family participation in the development, implementation, and ongoing modification of care plans.
  3. Participate in care and discharge planning for patients on admission, ensuring the care plan is established, implemented, and evaluated effectively.
  4. Coordinate nursing care for patients, set priorities, assign and delegate tasks to support staff, and make referrals to the interprofessional care team according to established protocols and practice standards.
  5. Collaborate with members of the interprofessional care team to identify and resolve issues, adjust care plans as required, and address changes in patient condition.
  6. Teach patients and families/caregivers about illness prevention, health restoration, and health maintenance to help patients achieve their healthcare goals.
  7. Assess learning needs of patients and families/caregivers, provide information and demonstration of care activities to support self-care and the discharge plan.
  8. Document assessments, nursing diagnoses, observations, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.
  9. Participate on nursing committees, care conferences, and councils as assigned or agreed to with the Manager.
  10. Participate in research and quality improvement activities by identifying patient care issues, collecting data, and identifying needs and recommendations for corrective action and changes to practices, procedures, or protocols.
  11. Participate in and attend in-services and other educational programs, identifying own learning goals and maintaining and updating current clinical competence and knowledge within the area of practice.
  12. Provide feedback to the Manager/delegate regarding the effectiveness of patient care programs, policy/clinical decision support tools, and equipment.
  13. Perform other related duties as required.
Qualifications

Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).

Completion of the applicable post-graduate specialty nursing course/program and/or willingness to complete the applicable post-graduate specialty nursing courses/program.

Current certification in Basic Cardiac Life Support (BCLS).

Ability to complete initial and ongoing patient assessment (clinical and diagnostic reasoning) and provide nursing care through appropriate/prescribed technical, therapeutic, safety-type interventions.

Ability to communicate effectively with patients, families/caregivers, the public, medical staff, and members of the interprofessional care team using verbal, written, and electronic communication means.

Ability to promote person-centered care that demonstrates empathy for patients and families/caregivers, sensitivity to diverse cultures and preferences, and patient advocacy.

Ability to integrate and evaluate data from multiple sources to problem-solve effectively.

Ability to teach and instruct patients and families/caregivers about topics essential to their health care and well-being.

Ability to organize and prioritize work.

Ability to collaborate with and promote cooperation among members of the interprofessional care team.

Ability to use factual information, prior learning, and basic principles and procedures to support nursing decisions and actions with relevant research-based evidence/evidence-informed practice.

Ability to operate related equipment, including relevant computer applications.

Physical ability to perform the duties of the position.
