Job Summary

We are seeking a highly skilled and compassionate Social Worker to join our team at CIUSSS du Centre-Ouest-de-l'Île-de-Montréal. As a Social Worker, you will play a vital role in evaluating the needs of our clients and developing personalized plans to support their well-being.

Key Responsibilities

  • Evaluate the needs of clients in relation to loss of autonomy due to aging or physical disability
  • Develop and implement individualized intervention plans in collaboration with interdisciplinary teams and external partners
  • Support the values and principles of home care, including the policy "At Home, the First Choice"

Requirements

  • Hold a Bachelor's degree in Social Work or Social Services
  • Be a member of the Order of Social Workers and Family Therapists of Quebec (OTSTCFQ)
