Job Summary

We are seeking a highly skilled and compassionate Social Worker to join our team at CIUSSS du Centre-Ouest-de-l'Île-de-Montréal. As a Social Worker, you will be responsible for providing clinical services to adults residing in a public mental health residential resource.

Key Responsibilities

  • Evaluate the social functioning of individuals based on their field of practice and skills, in accordance with the code of professions;
  • Conduct psychosocial evaluations of individuals within the framework of protection regimes or mandates;
  • Determine the use of restraint or isolation measures within the application of the Act respecting health and social services and the Act respecting health and social services for the Cree Nation;
  • Develop an interdisciplinary intervention plan or an individualized service plan and ensure its implementation by the various individuals involved, with the participation of the user and their loved ones, and partners, if necessary;
  • Sustain and restore the social functioning of individuals in reciprocity with their environment in order to promote the optimal development of the human being in interaction with their environment;
  • Perform individual or family interventions based on their field of practice and the service offer of the program;
  • Orient and refer individuals;
  • Maintain case files and perform statistical reports;
  • Collaborate with colleagues and supervisors;
  • Participate in program meetings and activities;
  • Work in an interdisciplinary manner;
  • Collaborate with internal and external partners;
  • Participate in the training of interns and research activities;

Requirements

  • Hold a first university terminal degree in social services;
  • Be a member of the Order of Social Workers and Family and Conjugal Therapists of Quebec;
  • Possess experience in the health and social services network with adult clientele in mental health and psychiatry;

Assets

  • Be trained in psychosocial evaluation within the framework of protection regimes;
  • Be trained in suicidal risk assessment;
  • Be trained in addiction screening tools (DépAdos and DépAdultes);
  • Be trained in the use of clinical information tools (OCCI);
