Hamilton Family Health Team (HFHT) is seeking a highly motivated and experienced Integration Director, Primary Healthcare to join our dynamic team. As the largest family health team in the province, we are dedicated to improving the health of Hamilton residents through innovative and collaborative care.

In this pivotal role, you will be responsible for spearheading the advancement of HFHT's strategic vision by establishing and executing four distinct neighborhood healthcare models across Hamilton. Your primary focus will be on ensuring a seamless network of healthcare services and fostering unified efforts among health and social service entities to enhance the patient experience.

Your Responsibilities:

  • Develop, implement, and assess strategic and operational plans for each neighborhood model.
  • Maintain exceptional clinical performance, operational efficiency, and high-quality patient care across all HFHT locations.
  • Cultivate strong collaborative relationships with stakeholders throughout the healthcare system, including physicians, nurses, social workers, community organizations, and government agencies.
  • Collaborate closely with the Primary Care Network (PCN) and Community Partners on projects aligned with HFHT's mission and vision.
  • Utilize data analytics to inform healthcare delivery strategies and plan for future workforce requirements.

We are seeking a candidate who possesses:

  • An undergraduate degree in a relevant health-related discipline (essential), with a master's degree preferred.
  • A regulated Health Professional designation and active membership in a professional college or association.
  • Minimum ten (10) years of progressive leadership experience in healthcare, with proven success in primary care settings.
  • A deep understanding of current healthcare challenges facing primary care providers.
  • Strong knowledge of best practices for advancing integrated care models.
  • Exceptional critical thinking skills to innovate and drive effective change within the organization.
  • Expertise in change management, process improvement methodologies, and Lean thinking principles.
  • Excellent communication and conflict resolution abilities.
  • Outstanding organizational, time management, and statistical analysis skills.
