Vancouver Coastal Health is seeking a dedicated Social Worker to join our Substance Use Assessment Team at Lions Gate Hospital.

In this role, you will play a vital part in supporting patients and families navigating substance use challenges. Your expertise will contribute to a holistic approach to care, emphasizing harm reduction and overdose prevention.

Your Responsibilities:

  • Collaborate with the interdisciplinary team to provide comprehensive support to patients experiencing substance use issues.
  • Conduct thorough assessments, offer referrals, and guide patients and families through available resources and systems navigation.
  • Provide case consultation to the team, sharing insights and best practices for supporting patients with substance use needs.
  • Contribute to discharge planning by addressing substance use concerns and ensuring a smooth transition back into the community.

Your Qualifications:

  • Current full registration with the British Columbia College of Social Workers.
  • Master's degree in Social Work from an accredited school of Social Work.
  • Two years of recent experience providing care within a holistic mental health, substance use, or primary care setting.

Essential Skills and Knowledge:

  • Strong understanding of systems navigation and community resources for individuals with substance use disorders.
  • Proficiency in conducting comprehensive assessments and developing individualized treatment plans.
  • Excellent communication and interpersonal skills to effectively collaborate with patients, families, and healthcare professionals.
  • Commitment to harm reduction principles and evidence-based practices in substance use care.
