Speech-Language Professional

We are seeking a highly skilled Speech-Language Pathologist to join our team at Home and Community Care Support Services. As a Speech-Language Pathologist, you will be responsible for providing speech-language pathology services to patients in their homes and communities.
Key Responsibilities- Assess patient communication and swallowing abilities to determine treatment needs and therapy goals.
- Develop and implement treatment programs to address established therapy goals.
- Oversee patients' progress and adjust treatment programs as needed.
- Provide education and training to patients, their families, and caregivers on how to progress with the treatment plan.
- Link patients with other in-house services or outside organizations as needed.
- Balance patients' needs and choices with available resources, ensuring their values and preferences are respected.
- Accurately document and chart patients' assessment results, goals, treatment plans, and interventions.
- Registered Speech-Language Pathologist in good standing with the College of Audiologists and Speech-Language Pathologists of Ontario (CASLPO).
- In-depth knowledge of speech-language pathology services and other community resources.
- Good understanding of the roles of other healthcare professionals.
- Relevant speech-language pathology experience in a community/healthcare environment.
- Comprehensive and up-to-date knowledge of speech-language pathology tools, processes, equipment, and assistive technology.
- Practical knowledge and understanding of relevant legislation regarding the provision of healthcare services, privacy, health, and safety.
- Comprehensive knowledge of the standards of practice and professional guidelines set forth by CASLPO.
- Ability to prioritize professional duties and manage multiple patients, and efficiently organize workload.
- Strong interpersonal and collaboration skills to work with diverse patient groups, healthcare professionals, community organizations, and service providers.
- Effective listening, observation, and facilitation skills.
- Ability to communicate information effectively through a variety of means.
- Working knowledge of computer software and Microsoft Office applications.
- Valid driver's license and access to a reliable vehicle.
- Valid Emergency First Aid Certification.
- Advanced oral and written proficiency in English.
- Experience working with diverse patient groups, including multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, and pediatrics.
- Case management experience or recent related community experience.
- Ability to speak French or another second language.

- Hourly wage of $43.23 to $50.00 as per the OPSEU Collective Agreement.
