Job Summary

We are seeking a highly skilled Speech-Language Pathologist to join our team at Home and Community Care Support Services. As a Speech-Language Pathologist, you will be responsible for providing speech-language pathology services to patients in their homes, community settings, and healthcare facilities.

Key Responsibilities
  • Assess patient communication and swallowing abilities to determine treatment needs and therapy goals.
  • Develop and implement treatment programs and compensatory strategies to address established therapy goals.
  • Oversee patients' progress and adjust treatment programs as needed to ensure therapy goals are met.
  • Provide education and training to patients, their families, caregivers, and substitute decision-makers on how to progress with the treatment plan.
  • Link patients with other in-house services or outside organizations as needed.
  • Balance patients' needs and choices with available resources, ensuring their values and preferences are respected.
  • Accurately document and chart patients' assessment results, goals, treatment plans, and interventions.
Requirements
  • Registered Speech-Language Pathologist in good standing with the College of Audiologists and Speech-Language Pathologists of Ontario (CASLPO).
  • In-depth knowledge of speech-language pathology services and other community resources, treatment centers, and organizations.
  • Good understanding of the roles of other healthcare professionals.
  • Relevant speech-language pathology experience in a community/healthcare environment.
  • Comprehensive and up-to-date knowledge of speech-language pathology tools, processes, equipment, and assistive technology.
  • Practical knowledge and understanding of relevant legislation regarding the provision of healthcare services, privacy, health, and safety.
  • Comprehensive knowledge of the standards of practice and professional guidelines set forth by CASLPO.
  • Ability to prioritize professional duties and manage multiple patients, and efficiently organize workload.
  • Strong interpersonal and collaboration skills to work with diverse patient groups, healthcare professionals, community organizations, and service providers.
  • Effective listening, observation, and facilitation skills.
  • Ability to communicate information effectively through various means, including meetings, reports, letters, and presentations.
  • Working knowledge of computer software (email, internet) and Microsoft Office applications (Word, Excel).
  • Valid driver's license and access to a reliable vehicle, as regular travel to the Espanola/Manitoulin Island, Elliot Lake region will be required.
  • Valid Emergency First Aid Certification.
  • Advanced oral and written proficiency in English essential.
  • We have a mandatory COVID-19 vaccination policy. As a condition of employment, all employees are required to submit proof of COVID-19 vaccination status prior to start date.
Preferred Qualifications
  • Experience working with diverse patient groups, including multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, pediatrics.
  • Case management experience or recent related community experience.
  • Ability to speak French or another second language.
