Responsibilities:- Provide leadership and guidance to a team of PSWs, ensuring they have the support and resources needed to deliver exceptional client care.
- Collaborate with clients, families, and other healthcare professionals to develop and implement individualized care plans that meet specific needs and goals.
- Conduct regular assessments of clients' needs and monitor their progress, making adjustments to care plans as necessary.
- Supervise, coach, and mentor PSWs, providing ongoing training and development opportunities to enhance their skills and knowledge.
- Ensure compliance with all relevant standards of practice, policies, and procedures.
- Address client concerns and complaints promptly and effectively, fostering a culture of open communication and transparency.
- Participate in quality improvement initiatives to enhance the delivery of care services.
- Registered Nurse (RN) or Registered Practical Nurse (RPN) with a minimum of 2 years of experience.
- Previous experience in a supervisory role within a community health setting is preferred.
- Solid understanding of principles, practices, and methods of community nursing and service delivery.
- Excellent communication, interpersonal, and problem-solving skills.
- Strong organizational and time management abilities.
- Valid driver's license and current CPR certification.
- Commitment to providing compassionate and person-centered care.
