Job Summary

Key Responsibilities
  • Provide Comprehensive Therapeutic Clinical Counselling Services
    • Explore the emotional psycho-social response to identify problems, personal concerns, and treatment objectives
    • Facilitate change in coping style, behavior, attitude, and feelings as it relates to the illness and other system issues
  • Conduct Comprehensive Social Work and Psycho-Social Assessments
    • Interview the patient and/or family, obtain relevant information, and gather social data regarding the patient and/or family
    • Interpret the data using a variety of modalities and in accordance with professional standards and clinical policies and practices
  • Educate and Provide Consultation
    • Provide education and consultation to patients, families, and healthcare professionals on issues related to high-risk factors, recovery, and current community practice relevant to the assigned clinical specialty area
  • Maintain Patient Records
    • Document patient assessments, charting patient information, preparing progress notes, and reports in accordance with established standards, policies, and procedures
  • Provide Leadership to the Interdisciplinary Team
    • Provide leadership to the interdisciplinary team in care planning, including discharge planning and hospital stay
    • Articulate analysis and decision to members of the interdisciplinary team
  • Communicate with Community Health and Social Agencies
    • Communicate with community health and social agencies, including government Ministries, Continuing Care personnel, and family physicians
  • Act as a Resource to Patients and Families
    • Provide information for patients and families regarding community resources and access to these resources
    • Suggest contacts to facilitate optimal planning
  • Participate as a Member of Local Committees and Planning Bodies
    • Participate as a member of local committees and planning bodies to identify areas for improved efficiency and effectiveness in service delivery
Qualifications

Masters Degree of Social Work from an accredited school of Social Work plus two (2) years' recent related experience. Current full registration with the BC College of Social Workers.
