We are seeking a highly skilled and compassionate Casual Social Supports Coordinator to join our team at the Government of Prince Edward Island.
About the RoleThe Social Supports Coordinator will provide direction and case management in delivering a range of specific supports and services to eligible persons and families. The programs to which the Supports Coordinator would be responsible to deliver are: Social Assistance; Child Care Subsidy; and Seniors Independence.
Key Responsibilities- Collaborate with clients, their families, and support networks to ensure case plans focus on maximizing opportunities for full citizenship.
- Provide screening and initial application supports for clients requiring eligibility assessments.
- Complete home and facility visits to promote a person-centered approach in supportive service provision.
- Contribute to a psychologically safe work environment in a supportive and positive manner.
- Provide specialization in assessing and delivering services using a collaborative case management model, with knowledge and expertise on support needs of people with multiple barriers.
- Facilitate the development of personalized services and support plans for individuals and families requiring Social Assistance; Child Care Subsidy or Seniors Independence.
- Implement person-centered collaborative case management by combining efficiency, commitment, care, and compassion.
- Determine eligibility in accordance with Program Act/Regulation; policies and procedures.
- Provide leadership in developing and implementing support plans to identify and address complex support needs for individuals and families at risk.
- Analyze information to confirm accuracy and evaluate the need for follow-up in determining eligibility for service, including closure and/or denial of service.
- Utilize the motivational interviewing approach to strengthen a person's motivation and commitment to change.
- Communicate effectively with clients, families, the general public, government agencies, non-government organizations, service providers, and funding agencies.
- Apply risk management approaches to recognize potential problematic and volatile situations and take measures to avoid crisis situations.
The ideal candidate will possess strong communication and interpersonal skills, with the ability to work effectively in a team environment. They will also have a strong understanding of the Social Assistance; Child Care Subsidy; and Seniors Independence programs, as well as experience in case management and support planning.
