Ensure the rights and choices of the individuals we support are respected and advocate as needed.

Effective Communication

Interact effectively with the individuals we support, their families, and community members.

Community Integration

Empower the individuals we support to be full citizens of their community, promoting independence and self-determination.

Personalized Support Plans

Participate in the development, implementation, and documentation of Personal Service Plans, ensuring the unique needs of each individual are met.

Relationship Building

Foster positive relationships with individuals, families, professionals, and community members, promoting a sense of belonging and connection.

Record Keeping

Record daily activity and personal health information (PHI) of the individuals we support on our case management database.

Home Maintenance

Work with individuals to ensure their home is clean and well-maintained, including daily food preparation and light housekeeping duties.

Requirements

  1. Education and Experience

Community Support Worker Diploma or equivalent, with a minimum of 2 years of experience in community social services, healthcare, or caregiving.

Certifications

Current Emergency First Aid/CPR and Food Safe certificate, previous behavior management course/training, and moderate computer literacy.

Clearance

Criminal record clearance for children and vulnerable adults, and proof of COVID-19 vaccination status as per current Public Health Orders.
