Vancouver Coastal Health is seeking a skilled Community Health Nurse to join our Home Care Nursing team at Sunshine Coast Home Care Services in Sechelt, BC.
Key Responsibilities- Provide high-quality nursing care to clients with acute, chronic, and palliative care needs.
- Work in partnership with clients and families to establish goals and develop care plans.
- Assess service eligibility and facilitate the development, implementation, and adaptation of care plans.
- Deliver direct care and provide care management to assist clients in managing their own care and navigating community services.
- Collaborate with other healthcare professionals and community partners to provide holistic care.
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- One year of recent related clinical experience in a relevant clinical area, working with complex clients/patients with chronic health conditions and/or functional mobility issues.
- Valid BC Driver's License required for local area travel.
