LPN, Hope to Health Primary Care Clinic

We are seeking a skilled Licensed Practical Nurse to join our team at the Hope to Health Clinic, a unique and innovative health clinic in the Downtown Eastside. As a LPN, you will be part of a multidisciplinary clinical team providing primary and specialty care to patients and clients in the DTES.
About the RoleAs a LPN, you will be responsible for providing nursing care to an assigned group of patients and clients, according to established policies and procedures. Your duties will include conducting assessments, implementing and evaluating interventions, documenting information, and acting as an advocate for patients and clients.
Responsibilities- Provide safe and respectful nursing care to patients and clients, according to established policies and procedures.
- Conduct head-to-toe assessments, monitor and recognize abnormal signs or symptoms, and communicate with the RN/RPN as required.
- Communicate with physicians regarding assigned patients and clients, and carry out verbal and/or written orders as required.
- Document care provided, including assessments, status, progress, treatments, changes in condition, medications administered, and discharge planning.
- Prepare patients for, and/or receive patients from, surgical and/or diagnostic procedures, transfers, and admissions.
- Administer medications, including narcotics and excluding intravenous route, according to established procedures.
- Use evidence-based resources to keep nursing knowledge and procedures up-to-date.
- Respond to incidents such as Codes, injuries or falls, hazardous spills, aggressive behavior, and fire alarms.
- Assist with the orientation of new staff and student preceptors.
- Identify patient and client and their significant other's actual and potential strengths and limitations, including physiological, psychological, socio-cultural, and spiritual needs.
- Encourage involvement of patient and client and their significant other in decisions that affect them.
- Identify and respond to patient and client and family learning needs, provide educational and reference materials, and incorporate patient and client and family teaching activities into care plans and nursing interventions.
- Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training, and experience.
- Registration with the British Columbia College of Nurses and Midwives as a practicing LPN registrant.
- Current basic cardiac life support certification.
- One-year recent, related experience working in an urban health setting, including experience with addiction, HIV/AIDS, or indigenous patient populations.
