Clinical Pharmacist

The Pharmacist is a key member of the healthcare team at Lakeridge Health, responsible for providing expert medication management services to patients. This role requires a strong clinical background, excellent communication skills, and the ability to work effectively in a fast-paced environment.
Key Responsibilities- Provide direct patient care, including medication reconciliation and counseling
- Participate in patient education and training programs
- Facilitate medication management activities and support the drug distribution system
- Collaborate with healthcare teams to ensure safe and effective medication use
- Conduct research and provide drug information to patients and healthcare professionals
- Current registration in good standing with the Ontario College of Pharmacists
- Practical knowledge of pharmacology and current best practice clinical guidelines
- Strong clinical assessment skills and ability to identify and solve drug-related problems
- Excellent medication counseling skills and ability to communicate effectively with patients and healthcare teams
- Ability to work effectively in an interprofessional team and demonstrate excellent organizational skills
- Proven ability to exercise independent judgment and assess situations and problems efficiently and effectively
- Terms and conditions of employment in accordance with the Collective Agreement between Lakeridge Health and OPSEU Local 348
- Successful external applicants must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past six months
