Job Summary

Lakeridge Health is seeking a highly skilled and dedicated Pharmacist to join our team. As a key member of our healthcare team, the successful candidate will be responsible for providing exceptional patient care and medication management services.

Key Responsibilities
  • Provide direct patient care and medication counseling services to patients and their families.
  • Work collaboratively with the healthcare team to ensure seamless patient care and medication management.
  • Participate in patient education and medication reconciliation activities to promote safe and effective medication use.
  • Teach and mentor students and healthcare professionals on medication management and patient care best practices.
  • Support the development and implementation of medication management policies and procedures.
Requirements
  • Current registration in good standing with the Ontario College of Pharmacists in Part A of the register.
  • Practical knowledge of pharmacology, current best practice clinical guidelines, and ability to interpret lab values.
  • Strong clinical assessment skills in identifying and solving drug-related problems.
  • Excellent medication counseling skills and ability to communicate effectively with patients and healthcare professionals.
  • Demonstrated ability to work effectively within an interprofessional team and exercise independent judgment.
  • Previous hospital experience desired.
  • Demonstrated commitment to Lakeridge Health's core values of Compassion, Innovation, Courage, and Trust.
Working Conditions

The successful candidate will be required to work in a fast-paced healthcare environment and must be able to adapt to changing priorities and deadlines. A satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past six (6) months, will be required for external applicants.
