About the Job

VON Canada is seeking a Licensed Practical Nurse to join our team in a Home Care and Visiting Care Agency setting. As a LPN, you will be responsible for providing high-quality patient care and support to individuals in their homes.

Key Responsibilities
  • Education and Training: Provide education and training to patients and their families on various health-related topics, including disease management and prevention.
  • Health Assessments: Conduct thorough health assessments to identify patients' needs and develop personalized care plans.
  • Medication Management: Administer medications and monitor patients' responses to ensure safe and effective treatment.
  • Wound Care: Provide wound care and management services to patients with various types of wounds.
  • Communication: Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements
  • Education: Diploma or certificate in Practical Nursing from a recognized institution.
  • Experience: 1-2 years of experience in a Home Care and Visiting Care Agency setting.
  • Certifications: Current CPR and First Aid certifications.
  • Language: Fluency in English.
