We are seeking a highly skilled and compassionate Health Services Practitioner to join our team at Norview Lodge in Simcoe, ON. As a key member of our interdisciplinary healthcare team, you will provide comprehensive primary care to our residents, including assessments, medication management, and palliative care.
Key Responsibilities- Collaborate with the Medical Director/Physician and Manager of Nursing and Personal Care to provide resident care.
- Participate in various committees, including Infection Prevention and Control, Medical Advisory, and Continuous Quality Improvement.
- Conduct person-centred health assessments, including three-month medication reviews.
- Provide after-hours and on-call coverage, advocating for residents' needs.
- Seek consultation with the Medical Director/Physician when necessary.
- Perform admission and annual resident examinations, documenting findings and results.
- Order and perform screening and diagnostic investigations, interpreting results and following up as needed.
- Manage resident care, providing pharmacological, complementary, and counseling interventions within the NP scope of practice.
- Document clinical data, assessment findings, diagnoses, plans of care, and resident responses in a timely and accurate manner.
- Participate in resident care conferences and increase continuity of care through collaboration and consultation.
- Registered Nurse with a current Certificate of Competence with the Ontario College of Nurses-Extended Class.
- University Degree in Nursing.
- Two years' experience in a long-term care setting, demonstrating strong clinical and assessment skills.
- Courses specific to gerontology, dementia studies, palliative care/end of life care, and wound care are assets.
- Working knowledge of Ministry of Health and Long-Term Care regulation standards, Fixing Long Term Care Act 2021, and relevant legislation.
- Strong knowledge of computer software, including PointClickCare, Excel, and Microsoft Word.
- Proven time management and organizational skills.
- Resident and customer focus.
- Effective oral and written communication skills.
- Excellent interpersonal skills.
- Ability to work independently and as part of a team.
- Ability to facilitate and adapt to changes in the long-term care environment.
- Current Basic Cardiac Life Support (BCLS) certification.
- Ability to perform Heimlich Maneuver.
- Valid Ontario driver's license and access to a reliable vehicle.
- Computer expertise in corporate standard software and department-specific software.
This is a temporary full-time position, reporting to the Manager, Nursing and Personal Care. The successful candidate will be part of a dynamic team, working in a fast-paced environment that requires adaptability and a commitment to excellence.
