Job Summary:

The Health Services Practitioner will be responsible for providing comprehensive primary care to residents in a long-term care setting. This includes conducting health assessments, developing care plans, and collaborating with the interdisciplinary team to ensure high-quality care.

Key Responsibilities:

  • Conduct comprehensive health assessments of residents on admission, annually, and as needed.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team.
  • Provide direct care to residents, including medication management, wound care, and palliative care.
  • Collaborate with the interdisciplinary team to ensure continuity of care and address resident needs.
  • Participate in resident care conferences and contribute to the development of care plans.
  • Document resident data, assessment findings, diagnoses, and care plans in a timely and accurate manner.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.

Requirements:

The ideal candidate will possess a current Certificate of Competence with the Ontario College of Nurses-Extended Class, a University Degree in Nursing, and two years of experience in a long-term care setting. Strong knowledge of Ministry of Health and Long-Term Care regulation standards, Fixing Long Term Care Act 2021, and relevant legislation is required. Excellent communication and interpersonal skills are essential for this role.

Benefits:

This is a temporary full-time position with a salary range of $102,771 - $128,467 per annum. The successful candidate will be required to work independently as well as part of a team, and will be expected to maintain confidentiality and adhere to infection control policies and procedures.
