Nurse Practitioner

We are seeking a highly skilled and compassionate Registered Practical Nurse (RPN) to join our team at Southbridge Care Homes in Goderich, Ontario. As an RPN, you will play a vital role in providing high-quality care to our residents, ensuring their physical, emotional, and social needs are met.
Responsibilities:- Provide direct care to residents, including administering medications, wound care, and vital sign monitoring.
- Develop and implement individualized care plans, working collaboratively with the interdisciplinary team.
- Conduct thorough assessments, identifying residents' needs and preferences, and making recommendations for care.
- Communicate effectively with residents, families, and healthcare professionals to ensure seamless care transitions.
- Participate in quality improvement initiatives, contributing to the development and implementation of best practices.
- Current certificate of competence from the College of Nurses of Ontario.
- Registration or eligibility for registration with the Registered Practical Nurses Association of Ontario.
- Comprehensive knowledge of nursing and healthcare practices, including infection control and medication management.
- Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
