Your Responsibilities:
In this role, you will deliver clinical pharmacy services to patients in both acute and residential care settings. As an integral member of a multidisciplinary team, your duties will include:

  • Assessing and recommending tailored treatment plans for patients.
  • Serving as the primary resource for drug information and education.
  • Providing expert guidance to healthcare professionals regarding medication management.
  • Monitoring drug prescribing, distribution, and therapy to ensure safe and effective medication use.
  • Maintaining accurate workload records and statistics.
  • Practicing patient-centered care by overseeing ongoing drug therapy.
  • Communicating significant findings and recommendations to relevant healthcare practitioners.
  • Interpreting medication orders and entering them into the computer system.
  • Dispensing prescriptions and resolving drug therapy issues.
  • Consulting with physicians on matters related to drug dosage, allergies, interactions, and efficacy.

Qualifications:
To be considered for this role, candidates must possess:

  • Current registration as a pharmacist with the College of Pharmacists of British Columbia.
  • Certification of completion of an Accredited Canadian Pharmacy Residency program (ACPR1); OR
  • A combination of at least one year of full-time equivalent (FTE) recent relevant clinical pharmacy experience and recognized advanced education courses; OR
  • A combination of at least three years of full-time equivalent (FTE) recent relevant clinical pharmacy experience and an equivalent combination of training, knowledge, and experience.
  • Active membership in the Canadian Society of Hospital Pharmacists is encouraged.
