Your Responsibilities:
This role involves delivering clinical pharmacy services to patients in both acute and long-term care environments. As an integral member of a multidisciplinary team, you will evaluate and recommend tailored treatment plans, serve as the primary source for drug-related information and education, and provide expert guidance to healthcare professionals. You will oversee medication prescribing, distribution, and therapy to ensure the safe, effective, rational, legal, and economical use of pharmaceuticals. Additionally, you will maintain detailed workload records and statistics, practice patient-centered care by monitoring ongoing medication therapy, and communicate relevant findings and recommendations to appropriate healthcare providers. Your duties will also encompass interpreting medication orders, entering them into the computer system, dispensing prescriptions, identifying and resolving medication therapy issues, and consulting with physicians regarding drug dosage, allergies, incompatibilities, interactions, and efficacy.

- Current registration as a pharmacist with the relevant regulatory body.
- Completion of an Accredited Canadian Pharmacy Residency program (ACPR1); OR
- A minimum of one year of full-time equivalent (FTE) recent relevant clinical pharmacy experience along with recognized advanced education courses; OR
- A minimum of three years of full-time equivalent (FTE) recent relevant clinical pharmacy experience combined with an equivalent mix of training, knowledge, and experience.
- Active membership in the Canadian Society of Hospital Pharmacists is encouraged.
