Job Summary

We are seeking a highly skilled and compassionate Social Services Coordinator to join our team at Home and Community Care Support Services. As a key member of our team, you will play a vital role in providing social work services to patients, promoting their psychosocial functioning, and supporting their independence.

Key Responsibilities
  • Respond to referrals from Care Coordinators to administer social work services to patients.
  • Assess patients to identify and determine psychosocial needs and therapeutic treatment needs and goals.
  • Provide treatment programs, counseling strategies, and emotional/mental health support to patients.
  • Oversee patient progress and adjust treatment programs as needed to ensure therapy goals are met.
  • Document and chart patient assessment results, goals, treatment plans, and interventions.
  • Link patients with other in-house services or outside organizations as needed.
Requirements
  • Registered Social Worker in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
  • Relevant social work experience in a community/health care environment.
  • Comprehensive knowledge of social work interventions, practice methods, and professional skills.
  • Knowledge of other community resources, funding agencies, treatment centers, social service networks, and organizations.
  • Good understanding of the roles of other health care professionals.
  • Up-to-date knowledge of social work theories, practices, and procedures.
  • Ability to prioritize professional duties and manage caseload in a time-efficient manner.
  • Strong interpersonal skills.
  • Effective listening, observation, and facilitation skills.
  • Strong mediation, negotiation, and conflict resolution skills to respond or assist in emergency situations.
  • Working knowledge of computer software and Microsoft Office applications.
  • Valid driver's license and access to a vehicle, as regular travel within the Cochrane District will be required.
  • Advanced oral and written proficiency in English and French is essential.
Preferred Qualifications
  • Experience working with diverse patient groups, including multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, and pediatrics.
  • Case management experience or recent related community experience.
  • Ability to speak French or another second language.
