We are seeking a skilled Social Worker to join our team at Home and Community Care Support Services. As a Social Worker, you will play a vital role in providing social work services to patients, promoting their psychosocial functioning, and preventing unnecessary deterioration.
Key Responsibilities- Assess patients to identify and determine psychosocial needs and therapeutic treatment needs and goals.
- Provide treatment programs, counseling strategies, and emotional/mental health support to patients.
- Oversee patient's progress and adjust treatment programs when needed, to ensure therapy goals are met.
- Link patients with other in-house services or outside organizations, if needed.
- Balance patient needs and choices with available resources, ensuring their values and preferences are respected.
- Registered Social Worker in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
- Relevant social work experience in a community/health care environment.
- Comprehensive knowledge of social work interventions, practice methods, and professional skills.
- Knowledge of other community resources, funding agencies, treatment centers, social service networks, and organizations.
- Good understanding of the roles of other health care professionals.
- Up-to-date knowledge of social work theories, practices, and procedures.
- Ability to prioritize professional duties and manage caseload in a time efficient manner.
- Strong interpersonal skills.
- Effective listening, observation, and facilitation skills.
- Strong mediation, negotiation, and conflict resolution skills to respond or assist in emergency situations.
- Working knowledge of computer software (email, internet) and Microsoft Office applications (Word, Outlook).
- Must have valid driver's license and access to a vehicle, as regular within the Cochrane District will be required. Travel to other areas within the North East geography may also be required from time to time.
- Advanced oral and written proficiency in English and French is essential.
